Schedule Part‑time, up to 20 visits per week.

Responsibilities
Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment recommendations
Establish measurable goals and develop treatment plans to move patients toward maximum independence and/or function
Maintain communication among the treatment team, physician, patient, and families
Maintain thorough, timely documentation for proper billing
Supervise Physical Therapist Assistants and Aides in direct patient care and related activities according to state practice act
Qualifications
Minimum bachelor’s degree in Physical Therapy from an accredited program
New graduates may be considered in select markets based on program availability
At least two years of experience as a PT; community/hospice experience preferred
Valid license in state of practice
Current CPR certification
Salary USD $58.00 / Hour
