Hours & Compensation
35 hours per week / $24.00 per hour (actual compensation will be determined based on experience and other factors permitted by law).

Summary Serves as the primary support and case manager for adults with mental illness by assisting them to live as independently as possible in their chosen community.
Essential Duties and Responsibilities
Develop a recovery or personal service plan with each member that identifies personal needs and goals.
Mediates and advocates on behalf of members to obtain and keep housing, health care benefits, entitlements, etc.
Visits members in their residence, hospital, or jail on an as‑needed basis to help them live successfully and prevent homelessness.
Transports members to appointments and on errands when appropriate.
Oversees and monitors individuals’ and group activities both inside the program and in the community.
Provides coaching, counseling, mentoring, instruction, training, feedback, and positive reinforcement for members in skills such as money management, grooming, hygiene, meal preparation, public transportation and other independent living skills.
Devises and implements outreach strategies for members who tend to isolate themselves.
Applies motivational interviewing and harm reduction practices.
Supports individuals and team goals in community integration and employment.
Participates in staff and community meetings and in day‑to‑day program operations as needed.
Required to bill 85 hours per month into the electronic health care system.
Maintains all records and documentation needed for program compliance and accountability in accordance with MHALA standards and funding source requirements.
Other Position Requirements
High School Diploma or GED, plus at least two years of experience providing psychosocial rehabilitation services to a disabled population (preferred).
All billing and supporting documentation entered into MHALA’s EHR within 24 hours from the date of service delivery.
All required data collection documents (e.g., MORS, Determinants, KEC, IHOMS, HMIS) are submitted on or before the due date.
Demonstrates knowledge of community resources.
Function well in a team‑oriented approach to member services.
Valid CPR certificate.
Knowledgeable in Microsoft Word, Excel, and Outlook.
Flexible work schedule including some evenings or weekends; on‑call cell phone services when necessary.
Driving with a current Class C driver’s license, vehicle for transportation, proof of valid automobile insurance, and ability to be insured by MHALA’s liability insurance.
TB clearance and annual re‑testing required for continued employment.
COVID‑19 vaccinations or booster shots are not currently required, but may become a requirement in the future. Failure to comply with agency vaccine policy may result in termination.
Employment verification from three past employers within the most recent seven‑year period.
