Physical Therapist – Hardeeville, Beaufort, Hilton Head Island, SC
We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Beaufort area. If you have a passion for providing patient care, improving outcomes, and building community relationships, Your Health Org is the perfect opportunity for you.
Physical Therapist role performs visits in homes, assisted living facilities, independent living facilities, clinics, and via Telehealth. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8‑hour position (8:00AM‑5:00PM, Monday‑Friday).

Responsibilities
Perform visits in homes, ALFs, ILFs, clinics, and via Telehealth.
Obtain complete health and psychosocial history and record findings in a systematic format.
Perform complete physical assessment while providing health education and counseling.
Arrange referrals to other members of the health team and to community resources as needed.
Use appropriate reporting mechanisms as required by law, respecting patient dignity, confidentiality, and privacy.
Obtain appropriate authorization and consents for treatments, review patient medical diagnosis, prognosis, procedures, and clinical course.
Market and provide community education.
Qualifications
Successful completion of a Physical Therapy training program and certified thought the therapy state board.
A current license to practice in the state of SC.
Current CPR Certificate.
Previous experience in healthcare setting is preferred.
Must be able to work flexible hours and travel between offices, facilities, and other places.
Ability to utilize electronic documentation required.
At least one year experience in the specialty field you are applying for.
Experience and/or interest in Pelvic Health Preferred.
