Full Time position based in Longmont, CO.
Base pay:
$37.00/hr - $52.00/hr (based on experience)

Duties
Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans.
Deliver rehabilitative, skilled, and medically necessary treatment interventions in accordance with regulatory and clinical standards.
Administer procedures including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care.
Provide consultation and counseling to patients, families, caregivers, and other service providers.
Generate comprehensive discharge summaries ensuring compliance with regulatory and clinical requirements.
Qualifications
Physical Therapy license is required.
Open to Physical Therapists of all experience levels; new graduates welcome.
