Position Summary Serves the organization by working with customers to include: inpatients, outpatients, physicians, allied healthcare professionals and the community by providing services such as education, referrals, and screenings. Utilizes clinical expertise in managing diabetes patients by providing appropriate information, training, and resources while employing effective problem-solving techniques to achieve desired outcomes. Ensures that he/she is current with new diabetes treatment technology.
Responsibilities
Provides individual and group education to diabetes patients.
Assists with coordinating diabetes patient care throughout the continuum.
Serves as a clinical resource and consultant for issues related to diabetes.
Participates in the marketing of department personnel and services.
Collaborates with all disciplines to enhance clinical outcomes, reduce length of stay, improve resource utilization and ensure patient satisfaction.
Provides continuing education to patients, hospital staff and the community.
Participates in complying with regulatory standards and requirements for maintaining recognition status for the outpatient program as well as the advanced disease specific care certification for inpatient diabetes care.
Establishes a system for understanding and communicating effectively with physicians involved in diabetes care.
Maintains a state-of art approach to diabetes care and self-management training. Other duties as assigned.
Qualifications
Bachelors of Science Degree (Nursing) with current license or Bachelors of Science Degree in Food and Nutrition and successful completion of dietetic internship experience through Academy of Nutrition and Dietetics program.
Required: Current Nursing or Registered Dietitian by Academy of Nutrition and Dietetics, Licensed by the State of Texas.
Required: BCLS/AED certification for "Healthcare Provider" by the American Heart Association.
Minimum 5 Years Of Experience.
Preferred: Masters Degree /Nursing or Dietetics.
Preferred: CDCES (Certified Diabetes Care and Education Specialist) certification within two years.
