We are seeking a qualified Nurse Practitioner to provide patient care in the Gainesville area.
As a Provider, you will deliver exceptional care in patients' homes and collaborate with a multidisciplinary team.

8‑hour shift (Monday‑Friday, 8:00 am‑5:00 pm)

Responsibilities
Evaluate high‑risk patients and develop individualized care plans.
Coordinate care activities with the interdisciplinary team to ensure continuity of care.
Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
Participate in interdisciplinary team meetings to discuss patient progress and care coordination.
Foster effective communication and collaboration.
Meet staff efficiency expectations while providing high‑quality care in various settings (homes, facilities, telehealth, clinics).
Document care management activities accurately for regulatory compliance.
Provide patient education and resources related to conditions, treatment plans, self‑management strategies, and preventive care.
Offer support and guidance to patients and families.
Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
Monitor quality measures and gaps in care, identify improvement opportunities, and implement corrective actions.
Qualifications
Valid Nurse Practitioner, Physician Assistant, or Physician license in the state of employment in good standing.
Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
Strong communication, collaboration, and interpersonal skills.
Ability to evaluate high‑risk patients, develop care plans, and coordinate activities.
Commitment to delivering compassionate, patient‑centered care.
Strong organizational and time‑management skills; ability to multitask and prioritize.
Basic computer knowledge.
Strong written and verbal skills.
Leadership skills and ability to foster a cooperative work environment.
Ability to work flexible hours, travel between offices/facilities, and maintain a licensed, insured vehicle.
