Psychosocial assessments/BIMS/PHQ9/Trauma Induced Care
Attend Care plans/meetings
MOLST review
Knowledge of HCPOA/Advanced Directives, PASSAR/ID screens
Work with Interdisciplinary team regarding discharge planning, MDS completion, formulate individualized care plans
Provide resources PRN
Advocate for Seniors Aging in Place
Attend meetings as appropriate
