As a PT you will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patient’s needs and physician direction. You are responsible for delegation and direction of all discipline‑specific therapy services for those assigned patients, ensuring the design of functional treatment programs to allow for an effective return to community activities. You will serve as an educator of patients, families, students, co‑workers, and yourself, and perform other professional duties associated with patient care, development of discipline‑specific therapy services, and design of program delivery.
Key Responsibilities
Evaluates assigned patients and establishes appropriate treatment goals and programs.
Provides ongoing evaluation of patient status and goals, modifying the treatment program for timely progression.
Develops comprehensive goals for patient treatment to maximize outcomes and return to community activities.
Coordinates treatment program in conjunction with other clinical/therapeutic services and available community resources; attends appropriate clinical meetings to facilitate coordination.
Provides patient education, including development of progressive home programs with instruction for the patient, family, and caregivers to facilitate a safe and effective discharge.
Prepares for patient discharge, including ordering necessary equipment and recommendations for therapeutic services ensuring continuum of care.
Evaluates patient in actual or simulated home, school, work or community environments as necessary to facilitate return to community activities, making recommendations for needed adaptations or training.
Identifies and manages various levels of care necessary to ensure care plan delivery; delegates and supervises the utilization of PTAs and support personnel to maximize outcomes and ensure effective service utilization.
Minimum Requirements
Current state licensure or eligibility for state licensure as a Physical Therapist.
Certified BLS.
