Adolescent Case Manager PRN
– Southcoast Behavioral Health Hospital, Dartmouth, Massachusetts
Rates: $32.50 – $48.50/hr (based on experience and education)

We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co‑occurring mental health and substance use disorders.
Purpose Statement Manage the client/patient/resident’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents. Caseload of 12. Work closely with another Case Manager, two attending providers, nurses, and BHA’s on the unit to engage the patient in treatment and provide a safe and adequate discharge plan. Completing treatment plans, psychosocial assessments, and 2× weekly meetings with each patient. Collaborate with families and community providers including DMH to coordinate care and discharge plans.
Essential Functions
Provide case management and care coordination services for clients/patients/residents in the facility.
Coordinate continuum of care activities and ensure optimum utilization of resources, service delivery, and compliance with external agencies and referral source requirements.
Conduct psychosocial assessments and individual, family, and group sessions.
Perform duties related to coordinating treatment and discharge activities as needed.
Develop, implement, manage, and evaluate client/patient care plans.
Collect, correlate, and provide clinical data to the treatment team.
Convey medical criteria and clinical information between the insurance provider and treatment team as warranted.
Correlate clinical data for the business office as required.
Coordinate issues or activities relevant to the treatment team in a timely manner.
Education, Experience, and Skills
Bachelor’s degree in Social Work, Counseling, or related human service field required.
One or more years’ experience working in a behavioral health setting preferred.
Previous experience with the patient population served by the facility preferred.
Previous experience in case management and/or utilization review preferred.
Licenses, Designations, and Certifications
Current license required within the state the facility operates in, if required.
CPR and de‑escalation/restraint certification required (training available upon hire).
First aid may be required based on state or facility.
