Registered Nurse Qualifications
Active Registered Nurse (RN) license in Wisconsin
At least 1 year of recent home health nursing experience (within the past 3 years)
Proficiency with HCHB (Homecare Homebase) and OASIS documentation systems
Ability to meet productivity expectations of 30 points per week
Excellent communication and organizational skills
Valid driver’s license and reliable transportation for home visits
Home Health RN Responsibilities
Deliver skilled nursing care to patients in their homes across Neenah, Appleton, and surrounding Wisconsin communities
Perform comprehensive patient assessments, develop individualized care plans, and provide direct patient care
Accurately document patient care using HCHB and OASIS electronic health record systems
Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
Educate patients and families on disease prevention, health management, and post-acute care
Ensure compliance with all home health regulatory and organizational standards
