Position Summary As a Home‑Health RN, you will develop a plan of care in collaboration with patients, family or caregivers, physicians, and other disciplines. You will visit patients’ homes to monitor their conditions and provide skilled nursing care.
Responsibilities
Perform comprehensive assessments in patients’ homes and identify patient needs.
Develop a care plan with patient, caregiver, healthcare provider, case manager, and/or supervisor and document appropriately.
Assess the need for additional services and equipment on admission and inform the case manager and/or supervisor in a timely manner.
Assess the home environment and recommend changes to enhance patient care, promote safety, and prevent adverse effects.
Take verbal orders from the healthcare provider and document promptly.
Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients and caregivers.
Maintain high standards of clinical care and stay current on clinical and medication issues.
Update patients’ clinical records, including medication profiles, plan of care, OASIS, case conferences, progress notes, and re‑certification documentation as required.
Serve as a preceptor when needed.
Other duties as assigned.

Qualifications
Graduation from an accredited nursing program.
Current state RN license and CPR certification.
At least one year of clinical practical nursing experience outside of school/rotations; Home Health experience preferred.
Must have a vehicle, current driver’s license, and automobile insurance.
Strong interpersonal and communication skills.
Ability to make home visits and provide skilled nursing assessments and care.
Experience with Home Health and Home Care Home Base (HCHB) documentation preferred.
