The

New York licensed LCSW

collaborates with fellow treatment team members to ensure the provision of quality care tailored to the unique needs and care levels of each patient within an outpatient treatment setting.

Hybrid model, one day per week in office required if full-time, with offices located in Manhattan, Brooklyn, The Bronx, Long Island, and New Jersey.

Responsibilities:

Receive, review, and promptly respond to intake referrals upon assignment of the

worker's compensation

client case.

Conduct Bio/Psych/Social assessments, diagnostic impression, and formulate treatment plans under the guidance of a qualified Clinical Supervisor.

Meet for individual sessions with patients at a frequency determined by the ongoing assessment of progress in treatment

Complete and assist clients in aftercare and discharge planning

Provide case management services as needed, including referrals to outside resources when necessary

Ensure cohesion of all services documented within the clinical chart (including but not limited to services provided by case management, medical providers and the laboratory), and assists with compliance of various standards

Maintain accurate, thoroughly documented client records with a quality of documentation that meets all applicable standards in the Electronic Health Record (EHR).

Attend and participate in treatment team meetings as scheduled to ensure quality of care for clients

Attend staff development and general staff meetings

Communicate with Medical Director and other prescribers to ensure quality clinical services are provided to the client on a timely and as needed basis

Participate in and attend supervision/consultation with the Clinical Director

Collaborate with members of the multidisciplinary treatment team to ensure effective delivery of a full spectrum of clinical services

Comply with local, state and federal confidentiality laws and regulations

Collaborate with professionals both within and outside the agency, as appropriate

Demonstrate sensitivity and responsiveness to cultural differences

Abide by PAX Health code of ethics and model values

Requirements

Licensed Clinical Social Worker licensed in NEW YORK STATE

5 years' experience as a fully licensed LCSW.

Must be able to follow our model of 30 minute sessions with our Workers' Compensation clientele.

New York State Workers' Compensation Board authorization number upon acceptance of an offer, which is a prerequisite before commencing work.

Ability to work as part of a comprehensive community mental health team and to represent the agency in the community.

Familiarity with community support systems, health care and human service resources, court and police activities, etc.

Good organizational skills and administrative abilities.

Flexibility in days and hours available for scheduled work including weekends.

Knowledge of and the ability to adhere to a professional code of ethics.

Knowledge of and the ability to comply with the state's mental health code.

Appropriate professional registration/certification by state/national authority required.
