Silver Birch of Terre Haute is seeking a Wellness Nurse to join our Clinical Team. Silver Birch Nurses provide exceptional care and service to our residents. We are seeking a Licensed Practical Nurse (LPN) to create a fulfilling career focused on providing the best care to our residents.

Responsibilities
Manages medication orders and documentation in compliance with regulations
Follows up with service providers to ensure resident needs are met
Administers medications and treatments as ordered and documents
Initiates and documents care delivered and care coordination, including completing admissions, assessments, transfers, and discharges
Supports and educates residents and families on proper healthcare directed by the resident care plan and promotes compliance
Provides exceptional customer service
Oversight of QMA and CNA and support in resident care

Qualifications
High School Diploma/GED
