The Bristol Dementia Wellbeing Service is a joint venture between Alzheimers Society and Devon Partnership Trust. It provides Dementia services to people who are registered to a Bristol GP from assessment throughout the person's journey. With co-production at the heart of what we do, we work holistically with the person and their carer and support network to support the people of Bristol with Dementia to live well. An exciting opportunity has arisen within the service for Band 6 Senior Dementia Practitioner to join our innovative and award-winning service on a part-time basis (30 hours per week), although we welcome anyone seeking full time hours to discuss with us before applying. Working as part of an MDT and in collaboration, the successful applicant will work closely with a designated group of Care Homes or GP surgeries using a liaison model of delivery. This is an opportunity for someone with a professional registration with transferable skills and experience working in Dementia, Mental Health, or Physical Health. This may be from a perspective of General Nursing, Mental Health or Learning Disability Nursing, Physiotherapy, Occupational Therapy, or Social Work. This service provides all staff with opportunities for excellent training and development, supervision and appraisal and not least to make a significant positive difference to the lives of people living with dementia. Interviews are scheduled to take place on Wednesday 28th Jan 2026. Main duties of the job

Working in the community using a range of core competencies (dementia-focused) to work with the Care Home and/or GP to support the needs of people who present with a range of memory loss, behaviour changes and changes in cognitive functioning symptoms. As part of the service, you will provide access to credible specialist dementia expertise for GPs, Primary Care and Care Home staff and others who are requiring support and expert advice on meeting the needs of those persons with a diagnosis of dementia. It will include case management (design and co-ordination of care/support that is personalised and evaluated). The liaison with a wide range of statutory and non-statutory agencies is central to this role. You will contribute to the clinical leadership of the team, providing support and informal supervision for other members and participating in team and service development and evaluation. You will have access to supervision and appraisal to develop your own area of expertise to support the service to develop and meet the needs of the local community more effectively. This role will be allocated to a designated hub but will be flexible to work across the city of Bristol as required. The role will necessitate working some extended hours on our Access Point (Monday to Friday from 08.00-18.00). Qualifications

A professional mental health qualification and registration eg RMN, Social Work Degree or equivalent, BSc/Diploma in OT or Community Nursing Degree. Non Medical Prescriber and/or another recognised therapy qualification Post graduate qualification relevant to specialism Training to level 3 in Safeguarding Adults and children and young people: roles and competencies for Health Care staff Experience

An understanding of the Mental Health Act/ Mental Health Capacity Act requirements. Understanding of community structures and Health and Social Care Agenda for Older Adults Understanding of the Ageing Well principles in Dementia care Understanding of the organic presentations in Older People An understanding of the CPA Process Skills & Abilities

Substantial experience of managing clinical complexity Working within a community setting in either the statutory or voluntary sector. Experience of lone working and able to make clinical decisions independently Providing clinical supervision. Experience of working in a multi-disciplinary and multi-agency environment. Participation in user involvement in services. Experience of delivering model based psychological therapy Communication and Working Relationship Skills

Excellent communication skills including the ability to engage with people experiencing a wide range of needs in relation to dementia. Ability to communicate with a wide range of professionals, community services etc. from various settings. Able to produce well written reports.
