About the Position:
A skilled SNF/LTC facility seeks an experienced or newly graduated PT to join their team on a contract assignment. The PT will assess referred patients requiring rehabilitation, provide direct patient care, develop and administer treatment programs, set realistic goals, document and educate patients and families.
Responsibilities
Assess referred patients for rehabilitation services, determining medical condition, functional capabilities and limitations.
Establish and administer individualized treatment programs with goals tailored to patient capacity and tolerance, under physician direction.
Set realistic and achievable goals, document patient conditions, and educate patients/families on appropriate physical therapy methods.
Develop plans of care for each PT patient and provide skilled interventions per physician orders.
Evaluate and adjust care plans to meet therapy goals, collaborating with all disciplines to plan and evaluate team goals.
Qualifications
Graduate from an accredited school of physical therapy.
Current state license as a Registered Physical Therapist/PT or actively pursuing licensure.
Current CPR certification.
