RN Patient Care Manager – Home Health Optum, part of LHC Group, is seeking a Registered Nurse (RN) to oversee clinical services in our Home Health program.

Primary Responsibilities

Provide clinical services within the scope of practice as defined by state nursing laws, in line with the plan of care and in coordination with other health‑care team members.

Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and schedule start‑of‑care visits.

Coordinate determination of home‑health benefits, medical necessity, and ongoing insurance approvals.

Continuously assess patient needs and ensure care is individualized, appropriate, and reasonable; meet home‑health eligibility criteria and physician orders.

Review assessments and plans of care daily, consult clinicians with recommendations, and maintain workflow compliance.

Required Qualifications

Current unrestricted RN licensure in the state of practice.

Current CPR certification.

Demonstrated ability to function effectively in any home setting regardless of client demographics.

Valid driver’s license, vehicle insurance, and reliable vehicle or access to public transportation.

State Specific Requirements

One year of clinical experience.

RN licensure must have no restrictions.

Preferred Qualifications

Experience in home care.

Ability to work independently.

Strong communication, writing, and organizational skills.

Compensation & Benefits Salary ranges from $71,200 to $127,200 annually for full‑time employment.

In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase options, and 401(k) contribution—all subject to eligibility requirements.
