Overview Are you passionate about providing compassionate care to patients in need? Contiuum Hospice is currently seeking a Full Time Compassionate Hospice Aide (CHHA) to join our dedicated team.
Schedule: M‑F 8‑4:30p
Territory: Sonoma and Marin Area
Your role will be to work under the guidance of a registered nurse to provide essential services and ensure patients’ comfort and well‑being.

Hands‑on Clinical Onboarding Program
Responsibilities
Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients.
Assist with Personal Care: Support patients with bathing, oral hygiene, changing bedding, clothing, and shampooing.
Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction.
Provide Support for Families: Guide and support families in caring for their terminally ill loved ones.
Maintain Accurate Documentation: Document details to support high‑quality care and regulatory standards.
Qualifications
High School Diploma or G.E.D. required.
Valid Certified Home Health Aide (HHA) license.
Valid Certified Nursing Assistant (CNA) license.
Minimum of 6 months experience in hospice or skilled nursing facility preferred; hospice experience a plus.
Current CPR certification if state‑mandated.
Pay $27.00 - $29.00 per hour (Average Pay Range). Final compensation will be determined based on experience, qualifications, and responsibilities. The exact rate will be discussed and confirmed at the conclusion of the interview process.
