POSITION SUMMARY This job is ideally for a Registered Dietitian or Registered Nurse with experience in diabetes patient education.
JOB DESCRIPTION:
The Diabetes educator will coordinate with designated staff to effectively integrate medical nutrition therapy into the overall management of diabetes.
The Diabetes educator will provide all aspects of diabetes education to Type 1, Type 2, and Gestational diabetes patients
Works with adolescents and adults of all ages individually and in group settings
Monitors educational plan of care as well as help patients manage meal plans
Collaborates with referring providers to recommend and adjust medications as requested.
Trains patients on diabetes technology devices including insulin pumps and continuous glucose monitors.
Assesses, evaluates and maintains professional development and competency related to the competencies outlined in the position description.
Qualifications
At least 2 years of experience in diabetes education
Credential as a Registered Dietitian (RD) or Registered Nurse (RN)
Current state license to practice as an RD or RN
Certified Diabetes Educator (CDE) or actively working to achieve certification
Knowledge of insulin pumps, continuous glucose monitors is essential
