Patient Navigator - Behavioral Unit/PRN Baptist Health Care

Job Description The Patient Navigator evaluates psychiatric patients in the emergency department and makes recommendations for disposition. The role consults with the attending psychiatrist, evaluates patients throughout the organization as requested or urgently, and facilitates or recommends appropriate referrals or preventative services, treatment and support for populations with alcohol, drugs and/or substance abuse.

Responsibilities

Interview, gather, and interpret clinical data for clients to provide a thorough analysis of their current state.

Use interpretive summaries and assessments to provide a clinically supported DSM diagnosis.

Continuously assess treatment and recovery progress, and adjust treatment plans as necessary with client and significant others to ensure progress toward treatment goals.

Adapt evidence‑based practice to individual needs (e.g., solution focused brief therapy, other brief therapies, assessment and treatment of co‑occurring disorders, CBT, DBT, motivational interviewing).

Recognize and manage transfer of care and other critical points in treatment.

Complete and maintain documentation and productivity to agency and program standards, including those related to medical necessity.

Qualifications Minimum Education

Master's Degree in Health Care Administration, Science, or Social Work, or Doctorate in Psychology.

Minimum Work Experience

2 years of experience working with children over 5, adolescents and adults who have mental health, substance abuse and/or co‑occurring disorders or developmental disabilities.

Licenses and Certifications

BLS for Healthcare Providers (BLS) from the American Heart Association – required upon hire.

Non‑violent crisis intervention (CPI) – required within 90 days.

Ukeru User Certification (UKERU\_CERT) – required within 90 days.

Required Skills, Knowledge, and Abilities

Assessment, diagnosis, and disposition of behavioral health patients.

Knowledge of services available to behavioral health patients.

Computer skills.

Ability to multitask.

Good organization, communication skills (medium proficiency).

Customer‑driven.

Ability to handle confidential information.

Strong communication and organizational skills (high proficiency).

Effective informational communication.

Knowledge of medical terminology and records management.
