Reporting to the Chief Strategy Officer, the Intake Nurse is responsible for responding to all inquiries and managing the patient intake process for Home Care and Hospice Services, coordinates the referral process for new patients, and serves as a telephone liaison between the professional staff and patient’s physicians. Additionally, this position is responsible for responding to customer requests and concerns.

Record accurate and thorough referrals for all Agency services into the Electronic Medical Record (EMR), and refer to the program clinical manager appropriately.

Identify insurance coverage and record accurately.

Provide accurate information to customer making telephone inquiries regarding Home Care, Hospice and other agency services.

Triage and relay calls to meet each individual caller’s needs.

Communicate clearly with professional staff regarding expected “call backs” and relay information to physicians and others to take accurate messages.

Assure all telephone calls which require a manager’s attention are referred in a timely manner.

Maintain a positive working relationship with current potential referral sources and individual seeking information

Maintain a comprehensive working knowledge of community resources and assist referral sources in accessing community resources if indicated.

Work to resolve inter/intra agency communication problems that affect patient care.

Collect data on telephone calls as requested.

Provide support to patients and families as needed.

Maintain a working knowledge of Granite VNA’s contractual relationships, insurance plans and managed care managers for initial authorization of services

Other Job Functions Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.

Expected Hours of Work The team member will be scheduled to work Friday, Saturday, Sunday, Monday : 8 a.m. – 6:00 pm. 38 hours per week.

Work assignments may include holidays.

Requirements

Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). RN Degree required.

Bachelor’s Degree of Science in Nursing and at least two years of nursing experience

At least one year of medical/surgical and/or community health nursing experience preferred
