Position Summary: The Care Manager is responsible for the coordination and facilitation of patient care plans through collaboration with the provider, interdisciplinary team, patient and family. The role enhances the quality of patient care, optimizes clinical outcomes and patient satisfaction while managing the cost of care and drives performance improvements. Responsibilities
Coordinate the plan of care through collaboration with the provider, interdisciplinary team, patient and family. Facilitate the plan of care with the objective of enhancing quality, optimizing outcomes and satisfaction while managing cost and identifying areas for performance improvement. Accountable for a designated patient caseload, plan effectively to meet patient needs, manage length of service and promote efficient utilization of resources. Minimum Requirements
Education: Associate Degree in Nursing or higher. Licensure: Current registered nurse licensure by the NC Board of Nursing. Experience: 0-3 years of medical case management. Pay Range
Salary Range > $62,400.00 - $90,948.00. Offer will depend on career ladder placement. Other Information
Location: ECU Health Family Medicine and Primary Care Clinic (outpatient) – Pinetops, NC. Full‑time (40 hrs/week) Monday to Friday 8:00 am – 5:00 pm. No nights, weekends, or call.
