Care Manager, LTSS - Field travel in Milwaukee County, WI
Provides support for care management/care coordination in long‑term services and supports (LTSS). Works with multidisciplinary teams to coordinate integrated delivery of member care across the continuum for members with high‑need potential, ensuring progress toward desired outcomes and contributing to strategy for quality and cost‑effective member care. Responsibilities
Completes comprehensive member assessments within regulated timelines, including in‑person home visits as required. Facilitates waiver enrollment and disenrollment processes. Develops and implements care plans collaboratively with members, caregivers, physicians, and other health professionals. Monitors care plans, documents interventions, evaluates effectiveness, and recommends changes. Promotes integration of services, including behavioral health and LTSS, to enhance continuity of care. Assesses for medical necessity and authorizes appropriate waiver services. Evaluates covered benefits and advises on funding sources. Facilitates interdisciplinary care team (ICT) meetings and informal collaboration. Uses motivational interviewing and Molina clinical guideposts to educate and motivate members. Identifies barriers to care and coordinates assistance for psycho/additional, financial, and medical obstacles. Detects critical incidents and develops prevention plans. Collaborates with licensed care managers/leadership as needed. Travels locally 25‑40% of work time as required. Required Qualifications
At least 2 years of health‑care experience, including 1 year with persons with disabilities or chronic conditions in LTSS and 1 year in care management or a relevant medical/behavioral‑health setting. LPN or LVN licensure preferred; licensure required only if mandated by state contract or regulation. Active and unrestricted license in the state of practice (if applicable). Valid, unrestricted driver’s license with reliable transportation and adequate auto insurance. Knowledge of community resources. Ability to work in diverse settings and adapt communication style. Proactive, detail‑oriented, self‑motivated work style. Responsive in communication and calm under pressure. Professional relationship‑building skills. Time‑management, prioritization, and multitasking skills. Problem‑solving and critical‑thinking abilities. Strong verbal and written communication skills. Proficiency with Microsoft Office and online portals/databases. Preferred Qualifications
Certified Case Manager (CCM), LPN, or LVN with active, unrestricted state licensure. Experience with populations receiving waiver services. Benefits and Compensation
