Position Overview Full‑time Bilingual Case Manager – Behavioral Health Home program. Location: 233 Main St, New Britain, CT 06051. Monday – Friday. Salary starts at $20.00 per hour.
Position Summary The Case Manager will engage and link homeless clients with mental health or co‑occurring disorders to MAT services. The manager serves as a liaison between CMHA and community providers to coordinate clinical services, housing support, and individualized case management, ensuring stable community living.
Essential Responsibilities
Provides advocacy, support and guidance in contacting external organizations (e.g., Social Security, DSS, DCF, DMHAS).
Educates clients on accessing emergency and community resources to meet basic needs.
Teaches life skills necessary for independent living, including finance management, activities of daily living, nutrition, housing maintenance and repair, and task completion prompts.
Collaborates with other CMHA teams to ensure all client services are communicated to the whole treatment team.
Maintains timely documentation in accordance with CMHA policies, including progress notes, treatment plans, authorizations, consent to treat, activity logs, and case management notes. Maintains appropriate caseload as determined by CMHA and funder.
Performs other related duties as assigned.
Requirements
Bachelor’s degree in a mental health‑related field preferred or equivalent experience.
Valid driver’s license.
Will become certified in MANDT, First Aid, and CPR.
Participates in annual training courses, as assigned.
Must be bilingual Spanish speaking.
Preferred Qualifications
Basic to intermediate experience with Microsoft Office (Word, Outlook).
Experience or willingness to learn an Electronic Medical Record (EMR) system.
