Job Summary and Responsibilities The Nurse Navigator is assigned to a population of patients and provides individualized and comprehensive care to reduce barriers and ensure prompt access to care. Utilizing the nursing process, the Nurse Navigator performs clinical assessments, educates on disease processes, and manages symptoms to support patients through their care continuum. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to coordinate patient care, ensure continuity of care, and provide an exceptional patient experience. The Nurse Navigator initiates referrals to appropriate inter‑disciplinary teams for physical, psycho‑social, and spiritual support through all stages of the continuum of care. The nurse navigator also collaborates with the social worker and supports patient navigation and social supportive items. The nurse navigator will support quality care by participating in and supporting quality improvement activities. The role upholds and applies standards established by the American College of Surgeons to ensure continued accreditation as a Community Cancer Center.
Requirements
Must possess and maintain a current Registered Nurse license with the state of California.
Must possess and maintain a current Basic Life Support (BLS) certificate.
Three (3) years of recent clinical experience in Oncology.
Bachelor's degree or higher in nursing education, healthcare, community health or related field within the last 3 years.
Excellent communication skills written and oral; able to adjust to variable audiences and levels of education.
Excellent interpersonal skills and the ability to work with others across all departments.
Serves as a liaison to community organizations to articulate the mission of the Mercy Medical Center Cancer Center and our core values.
Ability to identify patient/family needs and then to coordinate and deploy resources to effectively match those needs. Ability to work on multiple projects at one time. Works under the direction of the standards established by the American College of Surgeons Commission on Cancer. Requires minimal supervision and ability to work independently.

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post‑hire. This ongoing competency is required through internal resources; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
