About the role The Smoking Cessation Coordinator will lead the implementation of Sabathani Community Center’s culturally responsive tobacco cessation initiative funded through MDH. This position is responsible for coordinating community engagement, cessation education, promotion, referrals, partnerships, and evaluation activities outlined in the approved work plan.
The Smoking Cessation Coordinator will ensure that cessation efforts are community-driven, culturally grounded, and aligned with evidence-based and equity-centered practices. This role will serve as the primary point of coordination across internal Sabathani programs, community partners, healthcare providers, and insurers to ensure effective implementation, tracking, and sustainability of cessation efforts focused on Black residents of South Minneapolis.

What you’ll do Program Implementation & Support
Lead implementation of all grant-funded cessation activities, including Listening Circles, Healing Circles, Messaging Circles, storytelling events, and community education efforts
Coordinate development and delivery of culturally tailored cessation campaigns in partnership with the communications Coordinator
Ensure all activities align with approved grant objectives, timelines, and milestones
Integrate cessation education and referrals into existing Sabathani programs (e.g., Food Shelf, Maternal Health Matters, Senior Services)
Community Assessment & Engagement
Convene and support a Community Advisory Group to ensure community voice and lived experience guide program activities
Support establishment and coordination of a Professional Advisory Board to provide guidance on culturally responsive cessation practices, evaluation, and sustainability
Facilitate ongoing engagement with Black community members, elders, youth, parents, and faith leaders
Cessation Education & Promotion
Coordinate culturally responsive cessation education strategies that reduce stigma and normalize quitting
Support training and coordination of Quit Champions (e.g., barbers, stylists, pastors, community health workers)
Promote cessation resources through trusted community settings, events, and outreach activities
Develop and coordinate a streamlined referral process connecting community members to cessation services
Build and maintain partnerships with clinics, hospitals, insurers, and community-based organizations
Support partner collaboration to ensure cessation services and training are culturally relevant and accessible
Evaluation & Data Support
Support implementation of a culturally responsive evaluation framework aligned with grant requirements
Assist with tracking participation, referrals, and outcomes
Collect and document community feedback, focus group input, and survey data
Collaborate with leadership and evaluation partners to summarize findings and inform program improvements
Maintain documentation related to grant activities, milestones, and deliverables
Assist with preparation of grant reports, internal updates, and funder communications
Participate in required training courses, technical assistance sessions, and partner meetings related to the grant
Other Duties as Assigned Qualifications Education and Experience
Bachelor’s degree in community health, public health, health education, human services, or related field required
Experience coordinating community-based health or prevention programs
Experience working with Black communities and/or culturally specific organizations
Experience building partnerships with community or healthcare organizations
Experience supporting outreach or health education initiatives
Knowledge, Skills and Abilities:
Knowledge and/or training with smoking cessation and social determinants of health
Strong facilitation, collaboration, and communication skills
Proficiency in MS Office, Salesforce, and data analysis tools preferred
Ability to work evenings/weekends as needed for community events
Additional Qualifications
Excellent communication and demonstrated writing proficiency
Knowledgeable of community resources and access points
Proven ability to listen attentively and empathize with others.
Ability to respond in a professional manner in all situations.
Ability to set work priorities and to evaluate and create solutions to work related problems.
Ability to maintain boundaries.
Licenses and/or Certifications
Valid MN Drivers’ License, access to reliable transportation and the ability to meet Sabathani’s driving record requirements.
