Position Summary Under the direct supervision of the PACE Clinical Pharmacy Manager, the PACE Pharmacist is responsible for a wide range of RX disciplines, services and processes provided through the PACE Center and PACE Clinics. The PACE Pharmacist will be responsible for: Polypharmacy comprehensive medication review, transitions of care medication reconciliation, Medication Compliance & Safety, CMS Fraud/Waste/Abuse, Part D quality and safety compliance activities, PACE PBM management, clinical liaison between contracted pharmacies and PACE Providers, and PACE IDT RX representation and participation.
Essential Functions of the Job
Coordinates and oversees all PACE Center and PACE Clinic RX activities, duties and functions including RX Care Planning and implementation of RX Regimens for PACE Enrollees as per the PACE IDT RX Specific Care Plans for each PACE enrollee.
In collaboration with the SD PACE IDT Team and PACE Providers, conducts Medication Care Management Planning and Medication Therapy Management Consultative services for all PACE Enrollees.
Works closely with SD PACE IDT Team(s) and team members to optimize PACE enrollee RX safety, compliance, and adherence; attends weekly PACE IDT team meetings to collaborate with other PACE team members in reference to RX solutions and recommendations regarding individual PACE enrollee drug needs and follow-up.
Works closely with the PACE PBM to ensure optimal and compliant PDE claims processing and claims adjudication to CMS; reviews PBM utilization, process, and benchmark reports on an ongoing basis to ensure accurate and compliant PACE Part-D claims and billings to CMS.
Works closely with Quality team, Utilization Manager, and Medical Director to implement FWA and Part-D Compliance processes, plans, protocols, systems P&Ps and reporting to ensure full CMS and Part-D compliance across the PACE Delivery Continuum; is available to work with Part-D compliance consulting firms to achieve full Part-D and RX FWA compliance for all PACE RX operations and activities.
Works closely with PACE Providers under Collaborative Practice Agreement to provide clinical interventions and recommendations.
Manages RX IT services, platforms, systems, and processes impacting the SD PACE Center and PACE Delivery Continuum to include Epic, Pharmastar PBM interfaces, Pioneer, and other applicable IT PACE systems as appropriate.
Provides Geriatric specific Medication therapy management (MTM) services to PACE enrollees. Evaluates medication regimens in older adults, tailored to the geriatric patient’s individual circumstances, including co-morbidities, goals of care, preferences, and ability of elderly to adhere to medications. Recognizes PACE Drug therapy in older adults involves unique considerations, such as: potentially inappropriate medications, polypharmacy, prescribing cascade, and the impact of medications on geriatric syndromes (e.g. falls, cognitive impairment).
Additional Duties and Responsibilities
Coordinates with internal San Diego PACE Pharmacy and other contracted pharmacies to ensure clinically appropriate, accurate and timely dispensing of medications for PACE participants.
Supervise and train PACE Pharmacy Clerks and Pharmacy Liaisons in daily functions to provide appropriate pharmacy services for participants.
Evaluate and create workflows for PACE Centers, Clinics, and contracted pharmacies to optimize communication and efficiency in system processes.
Other duties as assigned.
Job Requirements Education Required
A Doctoral degree in Pharmacy (PharmD) from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
Experience Required
Minimum of 1 year of clinical pharmacist experience.
Certifications Required
Must have current, valid, unrestricted Pharmacist license to practice in California.
BLS for healthcare providers.
Verbal and Written Skills Required to Perform the Job
Must have excellent oral and written communication skills.
Spanish preferred.
Technical Knowledge and Skills Required to Perform the Job
Proficient in Microsoft applications and use of electronic health data systems; EPIC experience preferred.
At a minimum level, have good computer literact including internet navigations and research skills.
Demonstrates ability to be culturally sensitive and respect diversity.
Work effectively with individuals of different cultures and socio-economic status
Passion for service
Self-starter and highly organized
Ability to prioritize and complete a large volume of work within strict time deadlines.
