Overview BJC Medical Group is a multi-specialty physician‑led organization of BJC HealthCare including over 600 doctors and advanced practice providers affiliated with top‑ranked hospitals in the Midwest. It has served in over 145 locations and 25 specialties across St. Louis, Missouri, and southern Illinois since 1994, providing high quality patient care.
Role Purpose The Ambulatory Care Manager (RN) provides clinical care management services to eligible patients, coordinating care to achieve desired health outcomes, improving self‑care abilities, and decreasing unnecessary cost of care. The position performs comprehensive needs assessment, identifies and addresses barriers to care, collaborates with patients, providers and care team to develop and implement a medically appropriate, cost‑effective plan of care.
Responsibilities
Utilize Motivational Interviewing techniques to activate patients in self‑management of chronic conditions, improving symptom management.
Identify, enroll and manage patients per established protocols, maintaining a caseload per department policies.
Collaborate with PCPs and specialists to develop and implement care plans, maximizing outcomes and facilitating wellness with periodic review.
Perform medication reconciliation, provide disease‑specific education, and use teach‑back to ensure patient understanding.
Provide resource management to improve patient experience and reduce unnecessary cost and utilization (right care, right place, right time).
Minimum Requirements
Associate’s Degree in Nursing.
2-5 years of registered nursing experience.
RN license.
Preferred Qualifications
Bachelor’s Degree in Nursing.
