Position We are seeking a qualified Nurse Practitioner (NP or equivalent) to provide high‑quality patient care in the St. George area. The role is a full‑time, salary‑based position with monthly bonus opportunities.

Service Area

St. George / Eutawville / Cottageville

Shift

12‑hour shift (7 days on, 7 days off)

Responsibilities

Evaluate high‑risk patients with complex medical needs and develop individualized care plans.

Coordinate care activities with interdisciplinary team to ensure continuity of care and optimal outcomes.

Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.

Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.

Foster effective communication and collaboration to facilitate seamless patient management.

Meet staff efficiency expectations while providing high‑quality care in homes, facilities, telehealth, clinics, and specialties.

Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.

Provide education and resources related to patients’ conditions, treatment plans, self‑management strategies, and preventive care measures.

Offer support and guidance to patients and families to enhance their understanding of healthcare options and promote active participation in care.

Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.

Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.

Qualifications

NP, PA or Physician with a valid license to practice in the state of employment; license must be in good standing.

Demonstrated experience in providing patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.

Strong communication, collaboration, and interpersonal skills; can work effectively within an interdisciplinary team.

Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.

Commitment to delivering compassionate, patient‑centered care and promoting a culture of excellence.

Strong organizational and time‑management skills with the ability to multitask and prioritize responsibilities effectively.

Basic computer knowledge.

Ability to manage and demonstrate effective leadership skills.

Must be able to work flexible hours and travel between offices, facilities, etc.; must be a licensed driver with an insured automobile in good working order.
