Manager of Care Management Orlando – AdventHealth
About the Role The Care Management Manager reports to the Care Management Facility Director and oversees daily operations and supervision of the care management team, coordinating care, ensuring compliance with CMS Conditions of Participation, implementing social services processes, managing data for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and discharge planning, as well as department engagement, customer experience, compliance, and employee evaluations.
Responsibilities
Coordinate care management processes and services
Oversee CMS Conditions of Participation compliance
Implement social services processes based on data management
Accountability for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and discharge planning
Department engagement and customer experience management
Ensure compliance with regulatory standards
Conduct employee evaluations and foster professional relationships with the interdisciplinary team
Qualifications
Bachelor’s degree required (Master’s preferred)
Accredited Case Manager (ACM) – EV Accredited Issuing Body
Certified Case Manager (CCM) – EV Accredited Issuing Body
Certified Social Worker (CSW) – Accredited Issuing Body
Clinical Social Worker License (LCSW) – EV Accredited Issuing Body
Licensed Baccalaureate Social Worker (LBSW) – EV Accredited Issuing Body
Licensed Master Social Worker (LMSW) – EV Accredited Issuing Body
Licensed Social Worker (LSW) – EV Accredited Issuing Body
