Position Summary Under the supervision of the Registered Nurse, this position provides direct patient care, maintains a safe and efficient environment, and communicates effectively with the treatment team.
Primary Duties Direct Patient Care
Carry out all aspects of patient care as directed by the Charge Nurse.
Observe when specialized services may be needed and communicate to the Charge Nurse/RN.
Perform ongoing observation of patients and milieu to ensure active treatment and participation.
Respond promptly to patient requests.
Report changes in patient condition or behavior, including agitation, high-risk behaviors, vital sign changes, and eating, sleeping, and toileting patterns.
Intervene appropriately with patients experiencing a behavioral crisis such as suicidal or threatening behavior, elopement, or other situations.
Initiate and/or participate in emergency medical care when indicated.
Assist patients with activities of daily living, including meals, grooming, ambulation, and toileting as needed.
Participate in implementing individualized care plans and report factors that may impede success.
Provide educational information for identified learning needs.
Facilitate educational/activity groups as assigned or when indicated using approved materials, recommending alternatives based on patient functioning.
Ensure compliance with infection control protocols and policies.
Shift Support
Review assignment sheet and program communication tools each shift.
Facilitate a therapeutic and safe environment.
Remain on-duty until coverage for the subsequent shift arrives and receives a verbal report.
Record and store all patient valuables per policy.
Ensure patients leave with all personal belongings, medications, and discharge information.
Perform environmental and safety rounds, complete check sheets, and report issues to the Charge Nurse/RN.
Comply with National Patient Safety Goals and provide immediate feedback if non‑compliance is identified.
Customer Interaction
Interact with patients, families, external entities, and staff professionally and respectfully.
Collaborate with all treatment team members to support the facility’s mission of exemplary patient care.
Preserve patient and family dignity and safety.
Documentation
Document medical records and reports timely, legibly, thoroughly, and in required format.
Document patient observations, education, and incident reports as required.
Other Duties
Other duties as assigned.
Qualifications
Education:
High School Diploma or equivalent.
Experience:
One year of acute inpatient psychiatric care experience preferred.
Certification/License:
CPI/CPR Certified per policy.
