Home Care Speech Therapist
at
Wellstar Health System
will provide age‑appropriate, rehabilitative speech therapy services in patients’ homes. The therapist collaborates with the interdisciplinary team to design, implement, and document individualized care plans that meet physical, emotional, developmental, psychosocial, and educational needs. Core Responsibilities
Direct patient care by evaluating, planning, and delivering speech therapy interventions in the home setting. Manage the client’s rehabilitative needs from admission through discharge, adjusting visit frequency per physician order. Perform therapeutic exercises, education, and self‑maintenance strategies to promote communication independence. Maintain accurate, timely documentation of skilled interventions in the EMR and communicate status changes to the physician. Participate in orientation and education of new employees and attend continuing education and in‑service sessions as required. Collaborate with the clinical supervisor and interdisciplinary team members to coordinate services and support patient recovery. Required Education
Master’s Degree in Speech‑Language Pathology from an accredited professional program. Required Licenses & Certifications
BLS – Provisional (Basic Life Support) BLS – Instructor Speech Language Pathologist certification (State of Georgia) Georgia Driver’s License Private Vehicle Insurance Speech Language Pathologist certification (State of Georgia) Required Experience
Minimum 1 year Home Health experience. Required Skills
Strong critical care assessment and rehabilitative skills. Positive leadership and problem‑solving abilities. Organizational and time‑management skills. Excellent written and verbal communication. Proficiency with home‑health electronic medical records and related software. Knowledge of home‑health operations, reimbursement, and regulatory processes. Seniority Level
