Responsibilities
Enable patients to spend quality time with loved ones and doing activities they enjoy
Provide direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager
Build trusting, compassionate relationships with patients and their families
Empower caregivers by educating them on care for their loved one
Collaborate with care team to provide comfort, peace, and dignity
