Job Title :Licensed Clinical Social Worker (LCSW)
Status :Non- exempt; PRN
Hours: Varies
Salary Range: $37.50-$50.50 (Based on Experience)
Conduct comprehensive psychosocial assessments for home health and hospice patients.
Develop individualized care plans in coordination with nursing, therapy, and hospice teams.
Provide counseling and support to patients and families coping with illness, decline, grief, and end-of-life needs.
Facilitate advance care planning, including goals-of-care discussions and completion of advance directives.
Identify and connect patients/families with community resources, financial assistance, and support programs.
Document all assessments, visits, and interventions per CMS, state, and agency standards.
Participate in interdisciplinary team meetings and case conferences.
Provide crisis intervention and ongoing emotional support as needed.
Ensure compliance with Illinois regulations and professional ethical standards.
Qualifications:
Experience in home health, hospice, palliative care, or medical social work preferred.
Strong communication, assessment, and documentation skills.
Ability to work independently in a community-based setting.
Valid driver’s license and reliable transportation.
