day/evening/night, every other weekend
The Pharmacist is responsible for providing medication management, clinical support and consultative services in a complex pharmacy operation. This position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement and regulatory compliance. This position may also participate in education of pharmacy students and residents.
Responsibilities
Serve as the Emergency Department Pharmacy Care Expert
Participate in all code situations: Code Blue, Code STEMI, Code Stroke, ECMO activations and Rapid Sequence Intubations
Expedite and coordinate the administration of medications used in the treatment of sepsis
Institute Antimicrobial Stewardship efforts in the ED
Reduction of risks factors influencing Clostridium Difficile infection
Order review and verification of emergent medications
Patient medication use review
Written medication order review for cost efficiency and clinical appropriateness
Discharge medication review and consultation for complex patients
Drug information resource (easily accessible to all ED and Observation staff)
Documentation/Research (medication use findings and pharmacy intervention findings)
Completion of inpatient medication consultation on high- risk medications
Mentor & preceptor (to students and residents of pharmacy, nursing, and medicine)
Assure compliance with all regulatory activities
Provide pharmaceutical care to hospital patients
Review new medication orders for appropriateness of therapy: including patient assessment, identification and documentation of drug therapy problems and resolutions, review of appropriateness of therapy and follow-up and discharge counseling for selected patients
Supervise patients on an ongoing basis and adjust medications and doses after consulting with the prescriber
Provide interpretation of physician’s orders, checking for formulary status, appropriate drug selection, dose and dosage form, drug-drug interactions, drug-nutrient interactions, drug duplication, potential for adverse events, appropriate route of administration, therapeutic substitution, IV to PO conversion, or other formulary or departmental initiatives
Connect with physician or nurse to insure timely clarification on any part of the order in question, and recommend alternatives that are on formulary, if non-formulary medication is prescribed.
Evaluate therapy for efficient patient outcomes while ensuring medication safety
Coordinate drug distribution in assigned areas.
Complete or review entry of medication orders in the pharmacy computer system
Participate in team meetings/discussions with clinical pharmacy specialists, pharmacy residents, pharmacy students and technicians.
Serve as a preceptor for students and/or pharmacy residents on rotation
Provide training and education to other staff members
Meet all role-related and age specific proficiencies. Align with all initial and ongoing proficiencies within the entity and department. Complete required learning relevant to the role.
Required
Pharm. D. or Bachelors in Pharmacy Eligible for MN Pharmacy License
Completion of a PGY1 residency with rotations in the emergency department/critical care areas OR 3 years’ hospital experience in critical care or clinical emergency department roles
Preferred
Completion of a PGY2 residency in emergency department or critical care area
Clinical emergency department pharmacist experience
