Key Responsibilities 1. Assesses patient nutritional status using documented medical information and established nutritional standards.
2. Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
3. Identifies inpatients at nutritional risk and documents recommendations.
4. Diagnose and develop intervention for patients with malnutrition
5. Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
6. Performs other duties as required or assigned.
