**Job Description Summary**Duties of Position:

The Assistant Clinical Manager ensures the quality of client care in accordance with the Agency’s objectives and policies through evaluation, planning, coordination, and implementation of the home health services provided, under the guidance of the Clinical Manager and/or Director of Nursing.

Responsibilities of Position:

1. Assists with the coordination and schedules the daily workload to utilize Agency personnel to their fullest capacity.

2. Assists with the daily supervision of nurses, therapists, and home health aides to ensure the quality of services and maintenance of standards.

3. Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status and other procedures in client records. Assures that patients’ needs are continually assessed as well as the development, implementation and individualized plan of care is updated as necessary.

4. Directs staff toward the appropriate use of community resources.

5. Assists with staff performance during their probationary period and annually with the Director of Nursing; provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.

6. Assists with the coordination of referrals by identifying and evaluating client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff. Ensures assigned personnel have demonstrated appropriate competency in skilled services required.

7. Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures, under the guidance of the Director of Nursing.

8. Visits clients in the home setting as needed.

9. Participates in audits and clinical record reviews as necessary.

10. Ensures an interdisciplinary team approach by each ordered discipline and participates in the planning and coordination of care/services, which is reflected in case conferences or patient communication notes.

11. Reviews each nurse's schedule for recertification, discharges, and aide supervisory visits.

12. Reviews weekly schedules submitted by nurses for changes or updates.

13. Performs other duties as directed by the Administrator or Director of Nursing.

14. Substitutes for other supervisory staff when requested by Director of Nursing.

15. Is knowledgeable of the Agency’s organization, functions, and services.

16. Complies with Agency dress code policy.

17. Participates in community related functions as necessary.

18. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests.

19. Maintains current TB testing (refer to Health Screening Policy).

Job Conditions:

Position requires the ability to communicate well orally and in writing. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds and to lift and/or carry up to 30 pounds. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition; to perform and demonstrate client care; and, to assess a client’s home that may not be wheelchair accessible. Position is required to be available during all operating hours. Occasional travel is required.

Qualifications:

1. Graduate of an accredited School of Nursing.

2. Registered Nurse licensed by the State.

3. Preferably one (1) year of experience as a professional nurse in a Home Health Agency.
