Schedule:
Full-Time | Monday–Friday | Onsite Nursing Home & Community House Calls

Role Summary The Nurse Practitioner will deliver primary care services within designated nursing home facilities and through scheduled house‑call visits to homebound patients in Manhattan. This role is ideal for an NP who enjoys independent practice, continuity of care, and serving a medically complex geriatric population.
Key Responsibilities
Conduct routine and acute patient evaluations in nursing home settings and during home visits
Develop treatment plans and manage chronic conditions
Collaborate with facility staff, physicians, and care coordinators
Maintain accurate documentation and follow regulatory guidelines
Provide compassionate, patient‑centered care to seniors and homebound individuals
Qualifications
Nurse Practitioner License – NY (Active)
Experience in geriatrics, nursing home care, internal medicine, or home‑based primary care preferred
Strong communication and clinical assessment skills
Ability to work independently and manage a mobile schedule
