Job Title: Outpatient Palliative Care Physician (MD/DO)
Position Summary
We are seeking a compassionate and skilled Outpatient Palliative Care Physician to provide comprehensive, patient-centered care for individuals with serious or life‑limiting illnesses. This physician will focus on symptom management, goals‑of‑care discussions, and coordination of care in an outpatient clinic setting, working collaboratively with an interdisciplinary care team.
Key Responsibilities
Evaluate and manage patients with serious or chronic illnesses in an outpatient palliative care setting.
Provide expert symptom management, including pain, dyspnea, nausea, fatigue, anxiety, and depression.
Conduct goals‑of‑care conversations and advance care planning, including discussions regarding prognosis and treatment preferences.
Collaborate with primary care providers, specialists, social workers, nurses, and other interdisciplinary team members.
Support patients and families with complex medical decision‑making and care coordination.
Refer and coordinate with hospice services and community resources as appropriate.
Maintain accurate, timely, and compliant medical documentation.
Participate in quality improvement initiatives, program development, and continuing medical education.
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
Completion of residency training in a relevant specialty (Internal Medicine, Family Medicine, Neurology, Oncology, or related field).
Board certification or eligibility in Palliative Medicine or primary specialty; fellowship training in Hospice and Palliative Medicine preferred.
Active medical license to practice in the state of employment.
Strong communication skills, empathy, and experience managing complex chronic illness.
Work Environment
Outpatient clinic setting with potential for interdisciplinary team‑based care.
Primarily weekday schedule with limited or no inpatient responsibilities.
Collaborative and supportive clinical environment.
Must maintain state certification and/or licensure.
