Base pay range $55.00/hr - $65.00/hr

Administrative Functions

Direct day‑to‑day function of nursing assistants in accordance with current rules, regulations, and guidelines that govern the long‑term care facility.

Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.

Periodically review the department’s policies, procedures, manual, job descriptions, etc., and make recommendations for revisions.

Meet with your assigned nursing staff as support personnel, in planning shifts’ services, programs, and activities.

Ensure that the Nursing Procedures Manual is current and reflects the day‑to‑day nursing procedures performed in this facility.

Conduct the Nursing Service Manual procedures and ensure all staff comply with them.

Make written and oral reports/recommendations concerning the activities of your shift as required.

Cooperate with other resident services when coordinating nursing services to ensure the resident’s total regimen of care is maintained.

Ensure that all nursing service personnel comply with their job descriptions.

Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.

Periodically review the resident’s written discharge plan and participate in updating it as required.

Assist in planning the nursing services portion of the resident’s discharge plan as necessary.

Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies as required.

Admit, transfer, and discharge residents as required.

Complete accident/incident reports as necessary.

Oversee licensed professionals in the process of Resident Assessment.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, and other documentation as necessary.

Charting

Complete and file required record‑keeping forms/chart upon resident admission, transfer, and/or discharge.

Encourage attending physicians to review treatment plans, record, and sign their orders, progress notes, etc., in accordance with established policies.

Receive telephone orders from physicians and record them on the physician order form.

Transcribe physician orders to residents’ chart/medex treatment/care plan as required.

Chart nurses in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response.

Submit accident/incident reports to the director as required.

Chart all reports of accidents/incident involving residents and follow established procedures.

Record new/changed diet orders and forward information to the Dietary Department.

Report all discrepancies noted concerning physician orders, diet changes, charting errors, etc., to the Nurse Supervisor.

Complete transfer forms in accordance with established procedures.

Ensure that unauthorized discharges are documented in the resident’s medical record.

Perform routine charting duties as required.

Drug Administration Function

Prepare and administer medication as ordered by the physician.

Verify the identity of the resident before administering medication/treatment.

Ensure that medication prescribed for one resident is not administered to another.

Maintain an adequate supply of floor‑stock medications, supplies, and equipment on hand to meet nursing needs; report requirements to the Nurse Supervisor.

Order prescribed medications, supplies, and equipment as necessary in accordance with established policies.

Ensure narcotic records are accurate for your shift.

Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.

Review medication cards for completeness and accuracy in transcription of physician orders and adherence to stop‑order policies.

Notify attending physicians of automatic stop orders prior to the last dosage.

Dispose of drugs and narcotics as required and in accordance with established procedures.

Treatment Administration Function

Consult with the nursing Supervisor or other licensed nursing personnel regarding dermatologic disorders of residents.

Collaborate with the Care Planning Team concerning assessment, evaluation, and planning of skin‑care treatments for residents.

Initiate requests for consultation or referral; respond to resident, physician, or nursing staff requests.

Examine residents and their records to distinguish normal from abnormal findings, recognizing when to refer to a physician for evaluation, supervision, or direction.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.

Implement and maintain established policies and procedures relative to skin‑care treatments; interpret these to physicians, family, and the public as appropriate.

Make written and oral recommendations to the attending physician, Medical Director, or nursing supervisor regarding resident status and care.

Identify, manage, and treat specific skin disorders and secondary lesions, including abrasions, foot problems, decubitus ulcers, bacterial, parasitic, viral skin infections, scaling, benign tumors.

Provide assessment and diagnostic services; perform evaluation using observation, inspection, and palpation techniques.

Complete weekly physical dermatologic examinations and record results.

Provide therapeutic services and confer with the Care Planning Team in development of the care plan.

Provide treatment within protocols and under physician supervision for dermatologic disorders as directed by the attending physician or Medical Director.

Ensure residents with decubitus ulcers receive appropriate prophylaxis and treatment such as daily inspection, turning, activity, diet, and maintaining a clean and dry bed.

Give direct physical and psychological nursing care in emergencies and maintain residents’ well‑being.

Collaborate with other health agencies and family members on admission, care, and discharge transitions as necessary.

Assist in resident rehabilitation activities, provide range‑of‑motion, ambulation, and emotional support; monitor physical therapy outcomes.

Provide preventive health services and recommend nutrition, prevention of skin disorders, etc.

Educate residents’ families on prevention techniques and overall health improvement.

Attend continuing education programs to stay current on profession and licensure requirements.

Assist in monitoring medication inventories, supplies, and equipment to ensure adequate skin‑care product supply.

Use personal protective equipment as appropriate (mask, gown, gloves, back brace) during lifting.

Participate in employee performance evaluations and provide recommendations for personnel changes.

Report staffing needs when personnel fail to report to work.

Report absentee call‑ins and review workforce for recommendations to the Nurse Supervisor.

Develop work assignments and assist in completion and performance of assignments.

Provide leadership to nursing personnel on your unit/shift.

Conduct daily rounds to ensure nursing service personnel perform assignments per standards; report issues to the Nurse Supervisor.

Ensure nursing auxiliaries are enrolled in approved training programs and under licensed supervision; inform residents, staff, and visitors of policies.

Maintain a positive environment and rapport across departments to meet residents’ needs.

Review complaints, grievances, and disciplinary actions with fairness and compliance with non‑discrimination policies.

Provide and receive nursing reports at shift change and report occupational exposures per policies.

Nursing Care Functions

Inform nursing personnel of new admissions, expected arrival times, room assignment.

Ensure rooms are ready for new admissions; greet and escort residents to their rooms.

Participate in orientation of new residents and families to the facility.

Make rounds with physicians as needed.

Requisition and arrange for diagnostic and therapeutic services per physician orders.

Consult with physicians regarding resident care, treatment, and rehabilitation.

Review resident charts for medication orders, diet, etc., and implement nursing objectives.

Ensure proper medication administration by nursing assistants and evaluate resident status.

Cooperate with social and activity programs in care scheduling.

Notify physicians of accidents or incidents and changes in resident condition.

Carry out restorative and rehabilitative programs, including self‑help and care.

Maintain treatment areas of nursing service in a clean and safe manner.

Administer services such as catheterization, tube feeding, suction, dressing changes, and other procedures.

Use restraints following established policies when necessary.

Obtain specimens for lab tests; take vital signs and monitor serious illness.

Check food brought by families for compliance with dietary allowances.

Ensure care follows the resident’s plan and preferences; check residents who cannot call for help.

Admit, transfer, discharge, and escort residents to pick‑up areas; follow post‑mortem procedures.

Staff Development

Participate in developing, planning, conducting, and scheduling in‑service training classes on job duties.

Implement and maintain an orientation program for new staff.

Standardize work methods and assist in training personnel on potential blood exposure tasks.

Assist Assistant Director with planning clinical supervision for nurse aide trainees.

Attend and participate in OSHA, CDC, and other training programs.

Maintain licensure status through continuing education.

Safety and Infection Control

Monitor personnel to ensure compliance with safety regulations and equipment use.

Ensure de‑identified policy compliance by nursing personnel.

Assist Director and Infection Control Coordinator in identifying, evaluating, and classifying routine and job‑related tasks for blood/bodily fluid exposure.

Ensure personnel have appropriate protective equipment available.
