Helps give care and support to terminally ill persons in various settings, working with a transdisciplinary team, and following the Nursing Process to implement the Plan of Care.
What You Will Do
Practice in accordance with HWR clinical standards, policies and procedures.
Assess physical, psychosocial, and spiritual needs of patients and family/caregivers.
Provide intermittent skilled nursing services under physician direction.
Monitor and evaluate pain and symptoms and provide information and assistance to manage the problems.
Serve as liaison to physicians when needed, providing information based on patient needs.
Provide emotional support to patients and families.
Refer patients/family to other services when appropriate and follow up.
Provide instruction (health teaching) to patient and family.
Maintain and promptly complete all necessary plans and reports related to hospice care.
Participate in care conferences and bereavement follow-up.
Participate in case review, quality improvement activities and other meetings as assigned.
Plan and clinically supervise activities of HNA related to patient care.
Participate in on-call rotation as scheduled.
Participate in discharge planning with team members.
Success Capabilities
Licensed in State of Ohio.
Experience preferred in hospice care, home health, oncology and/or pain and symptom management.
Current CPR certification.
Must have and maintain a valid driver’s license, good driving record and auto accident insurance acceptable to HWR.
Hospice and Palliative Care Certification desired; required within first two years of hire.
Equivalent education/experience may substitute for minimum qualifications except legal requirements.
