Vacaville, Solano County, CA
Base Pay Range:
$120,000 – $130,000 per year
Full Time – Occupational Therapist (Home Health):
You will partner with a Case Manager to evaluate and treat patients in their homes so they can live safely and productively.
Key areas include home safety and medication management, wheelchair seating and positioning, pain management and falls prevention, chronic disease and dementia management, and family caregiver training.
You will serve as an educator, patient advocate, and member of an interdisciplinary care team.

To Succeed as an Occupational Therapist (OT) Home Health:
Current unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Knowledge of individual development and human behavior as it relates to health care.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home health care (highly preferred).
Experience with high‑acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case Management fundamentals (highly desired).
Self‑directed, empathetic, critical thinker.
Outstanding interpersonal skills, leadership, organized.
