Job Description Under the supervision of a Registered Nurse, coordinates the Plan of Care for the patient in the home, hospital or other setting. Works cooperatively with other team members in maintaining standards of professional nursing practice and in accordance with age specific needs and within the scope of practice for Licensed Practical Nurse within New York State.
Location Home Care (VNA) – Genesee County, NY (Batavia)
Work Type Full-Time
Shift Shift 1 – 8:00 a.m. – 4:30 p.m.
Education & Credentials
Associate of Applied Science (AAS) degree in Licensed Practical Nursing (required).
Valid NYS license as a Practical Nurse (LPN) upon hire.
Valid NYS Driver’s license upon hire.
Experience One year of experience in Acute medical/surgical nursing is preferred.
Working Conditions
Weight Requirement – Medium (up to 50 lbs)
Weekend/Holiday Requirement – Yes
On-Call Required – Yes

Scheduled Work Hours – 8:00 a.m. – 4:30 p.m.
