Position Summary The Social Worker provides individual, group, family, and marital counseling to patients with a substance use disorder and their family members. The role performs intakes, develops treatment plans, and makes referrals as part of a multidisciplinary team while acting within the SB ELIH and Quannacut mission, philosophy, and goals.

Job Duties & Essential Functions

Conduct thorough intakes to identify problem areas, formulate initial diagnoses, and make treatment recommendations.

Develop initial treatment plans and monitor patient progress through ongoing reviews.

Assist and direct clients to complete necessary paperwork for entitlements.

Makes referrals to community resources for clients and family members.

Monitors client care through follow‑up with agencies and community resources.

Provide individual, group, family, and marital counseling focused on substance use and its impact on life areas.

Display knowledge of alcohol and other addictive drugs and their effects on human behavior.

Display knowledge of the disease concept, harm reduction, and factors impacting recovery.

Inform patients of their rights and responsibilities, including confidentiality and treatment recommendations.

Work through patient resistance and adverse reactions during treatment.

Foster an empathetic therapeutic relationship and engage patients.

Educate, elicit feelings, facilitate self‑understanding, and motivate patients.

Acknowledge denial and other defense mechanisms.

Utilize crisis intervention techniques for alcoholic/chemically addicted patients.

Assist in reframing and interpreting life experiences to address shame, guilt, and low self‑esteem.

Apply common psycho‑therapeutic techniques to augment recovery.

Identify underlying psychological and/or emotional issues warranting treatment focus.

Explore deep‑seated underlying issues.

Identify and cope with transference and countertransference.

Demonstrate skills essential to family and marital therapy.

Identify family impact and make appropriate outreach when needed.

Facilitate referrals as determined by the treatment team.

Locate and use information resources including literature, research findings, and consultation services.

Participate in multidisciplinary staff meetings and collaborate on treatment recommendations and plans.

Understand the history, philosophy, and process of recognized self‑help and 12‑step programs.

Communicate effectively (verbal and written) to healthcare professionals, community systems, patients, and lay persons.

Maintain accurate records in accordance with SB ELIH, QOPS, and OASAS policies.

Adhere to hospital and departmental rules and regulations.

Participate in in‑service programs.

Participate in annual recertification, compliance, and HIPAA training.

Perform all other related duties as assigned by management.

Qualifications Required Education & Qualifications

NYS LMSW, NYS LCSW, or LM SW, eligible to obtain certification within six (6) months.

BLS required.

Understanding of alcoholism/addiction and 12‑step programs demonstrated by a minimum of one (1) year of continuous personal, professional, or educational experience.

Preferred Qualifications

Experience in Emergency Department Social Work.

Experience in hospital, inpatient settings.

Discharge planning experience.

Bi‑lingual.

Physical Demands

Sufficient mobility to handle department activities.

Dexterity of hands and arms (or mechanical substitutes).

Adequate vision and hearing to write, answer phones, and communicate with staff, clients, and other departments or agencies.

Anticipated Pay Range The starting salary range has been established based on relevant experience. The range represents a good‑faith estimate of possible compensation at the time of posting. Your total compensation goes beyond the number in your paycheck.

Pre‑Employment Requirements

Complete pre‑employment physical examination and obtain medical clearance from Stony Brook Eastern Long Island’s Employee Health Services.

Complete electronic reference check with a minimum of three (3) professional references.

Successfully complete a 4‑panel drug screen.

Meet regulatory requirements for pre‑employment screenings.

Provide a copy of any required New York State license(s)/certificate(s).

Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer. The Human Resources department will be responsible for any fee incurred for examination.
