Details Location: Westdale Middle School
Hours: Monday-Friday 7:00 a‑3:00 p. (No holiday or summer shifts)
Responsibilities
Support the mental and physical health care of patients on an assigned caseload with the patient’s medical provider and consultant psychiatrist.
Screen and assess patients for common mental health and substance abuse disorders; facilitate patient engagement and follow‑up care.
Provide patient education about disorders and available treatment options.
Support psychotropic medication management as prescribed by medical providers, focusing on adherence, side‑effects and effectiveness.
Provide brief behavioral interventions using evidence‑based techniques such as behavioral activation, problem‑solving treatment, motivational interviewing or others as appropriate.
Document patient progress and treatment recommendations in EHR for the care team.
Facilitate treatment plan changes for patients not improving, in consultation with medical provider and psychiatric consultant, including referral to more intensive care outside the clinic.
Develop and complete relapse‑prevention self‑management plan with patients ready for discharge.
Systematically track treatment response and monitor patients (in person or telephone) for symptom changes and side‑effect complications.
Participate in regular weekly caseload consultations with the psychiatric consultant and communicate recommendations; case reviews may be by telephone, video or in person.
Track patient follow‑up and clinical outcomes using a registry; document in‑person and telephone encounters.
Ensure accurate and timely billing for therapeutic services.
Qualifications
2 years of experience as a Licensed Clinical Social Worker with knowledge of mental health therapeutic processes and counseling techniques.
Master of Social Work degree.
Working knowledge of evidence‑based psychosocial treatments and brief behavioral interventions for common mental health disorders (e.g., motivational interviewing, problem‑solving treatment, behavioral activation).
Licensed Clinical Social Worker (LCSW) license.
