PRN | In‑House Therapy Team
Position Overview The PRN Physical Therapist evaluates residents and develops individualized treatment plans to restore function, relieve pain, and improve mobility. This role works closely with residents, families, and the interdisciplinary care team to help patients achieve the highest level of independence possible.
Key Responsibilities
Plan and administer physical therapy treatments for residents with short‑term and long‑term conditions
Develop and implement individualized plans of care
Relieve pain, restore mobility, and improve functional independence
Provide education and guidance to patients, families, and caregivers
Monitor patient progress and adjust treatment plans as needed
Maintain accurate and timely documentation in compliance with company standards and state/federal regulations
Collaborate with nursing staff and other rehabilitation professionals to ensure coordinated care

Qualifications
Licensed Physical Therapist (PT) in the state of Pennsylvania
Graduate of an accredited Physical Therapy program
Strong communication and collaboration skills
Experience in skilled nursing or rehabilitation setting preferred, but not required
