Wound Care Specialist - NP or PA Part‑time role (1‑2 days/week) located in Sterling, CO. Qualified candidates will round at multiple facilities within a designated territory, performing wound care in skilled nursing facilities, long‑term acute care hospitals, and assisted living settings.

Base Pay Range

$48,048.00/yr – $57,200.00/yr

Qualifications

Active state professional license

Valid driver’s license

Duties

Weekday commitment: No call, evenings, or weekends

Deliver wound care at the bedside in the post‑acute care setting

Round daily at multiple facilities

Responsibilities

Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation

Collaborate with facility staff to ensure an effective wound care plan

Perform wound debridement and prescribe appropriate wound dressings

Educate nursing staff, maintain open communication with primary care physicians, families, and clinical teams as required

Order necessary medications and provide clear instructions for their use

Provide quality patient care to the geriatric patient population
