Position Summary Under a grant from the Office of Refugee Resettlement (ORR), NYAP’s Post Release Services (PRS) Intensive Case Manager Level 3 provides post‑release services aimed at stabilizing family units using trauma‑informed and intensive case management. The role connects families with community services and requires initial in‑home assessment followed by regular visits: weekly for the first 45‑60 days, then monthly (in‑person or virtual) for 6 months.

Responsibilities

Provide Level Three (3) post‑release services.

May provide Level Two (2) and Level One (1) services as needed.

Deliver intensive in‑home services and engage children and families.

Connect released child and sponsor family with community‑based services.

Maintain monthly contacts for 6 months following referral acceptance.

Carry a minimum caseload of 3 cases per worker.

Establish working relationships with community‑based organizations.

Use a case‑management approach that assesses, plans, facilitates, and advocates for unaccompanied children and sponsors.

Follow all ORR guidelines and policy requirements.

Demonstrate knowledge of ORR policies, Flores Agreement, and relevant laws including TVPRA of 2008.

Report to PRS Clinical Supervisor.

Perform other duties as assigned.

Minimum Qualifications

At least 21 years old.

Master’s degree in social work, psychology, sociology, or other relevant behavioral science with direct clinical experience requirement; or bachelor’s degree with at least 5 years of clinical experience. May be licensed or eligible for licensure.

Bilingual in English and Spanish.

Prior experience with ORR HS/PRS preferred.

Proficient in ORR’s Official System of Record (UC Portal).

Ability to travel up to 75% of the time; valid driver’s license, reliable vehicle, good driving record, and 100/300 insurance coverage on personal vehicle.

Pass criminal history screen, including state/local child protection registry and C/ANs check.

Willingness to work flexible and non‑traditional hours.

Live within 100‑mile radius of assigned city.

Travel required to states within assigned region.
