Job Description
The Case Manager acts as a patient advocate to hospital clients. This autonomous role coordinates, negotiates, procures services and resources, and manages the care of patients throughout the continuum of care, including arranging post-acute services. The Case Manager is responsible for team building, educating, and consulting interdisciplinary team members. This position requires a strong clinical background, preferably in an acute‑care setting, flexibility to work in all hospital areas, and adaptability to change and any assignment where needed. The role includes discharge planning, care coordination to facilitate quality health care, and identification of appropriate utilization of resources across the continuum. Responsibilities
Direct and coordinate case management for patients in caseload. Prevent delayed discharges of observation patients. Participate in case finding and pre‑admission evaluation screening to ensure a safe discharge plan. Complete case management assessment of patients and support systems to facilitate timely transition plans. Interact, communicate, and intervene with multidisciplinary healthcare team in a purposeful, goal‑directed manner; proactively maximize resource utilization and resolve issues related to resource use, continued hospitalization, and discharge planning. Maintain professional commitment to institution and department goals, demonstrating flexibility in floor, work schedule, and other internal and external demands. Stay current with case management and discharge planning guidelines as specified by federal, state, and private insurance regulations. Minimum Education
Technical Diploma/Certificate in Practical Nursing or Associates Degree in Nursing (required). Minimum Work Experience
3 years Acute Hospital Nursing Experience (preferred). 1–3 years Case Management in an Acute Care Setting (preferred). Licenses and Certifications
Registered Nurse licensed in the State of Florida or eligible compact license (required upon hire). Required Skills, Knowledge and Abilities
High proficiency in negotiation, communication (verbal and written), conflict resolution, interdisciplinary collaboration, management, creative problem solving, and critical thinking. Medium proficiency in knowledge of healthcare financing, community and organizational resources, patient care processes, data analysis, and post‑acute care community resources. High proficiency in adaptability to changing priorities and regulation; excellent verbal and written communication; proficient computer skills; effective documentation; excellent interpersonal skills; strong organizational and multitasking abilities; understanding of implications of new information for current and future problem‑solving and decision‑making; ability to plan, organize, and direct activities of others.
