Child & Adolescent In Home Therapist – Per Diem

$60.00/hr – $60.00/hr Position Summary
This is a Per Diem position for an Other Licensed Practitioner (OLP) provider who plays a key role within the Children and Family Treatment and Support Services (CFTSS) framework. The OLP provides specialized, community‑based clinical services to youth ages 5 through 21 and their families. The Per Diem OLP carries a minimum caseload of five youth and may not exceed nineteen payable hours per week. Responsibilities
Conduct comprehensive assessment, diagnosis, direct psychotherapy, and develop individualized treatment plans. Provide individual and family therapy tailored to each youth’s unique needs. Collaborate closely with caregivers, referring providers, and community supports to promote clinical progress. Use evidence‑based interventions for childhood mental health disorders and pursue ongoing professional development. Maintain accurate, timely records in the Electronic Health Record (Avatar) and comply with all regulatory standards. Participate in CFTSS team meetings, trainings, and supervision. Adhere to agency safety policies, confidentiality laws, and mandated reporter training. Travel to clients in the home and community settings; a personal vehicle is required. Qualifications
Licensed Mental Health Professional (LCSW, LMSW, LMHC, or equivalent). Minimum of one year of experience working with children diagnosed with Serious Emotional Disturbance (SED) and families. Valid New York State driver’s license with a clean record and reliable, insured transportation. Physical Demands
Occasionally stand, walk, sit, use hands, reach, climb stairs, and lift up to 25 pounds. Reasonable accommodation may be made to enable individuals with disabilities to perform these functions. Work Environment
