Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at [Facility Name], a long-term care nursing facility. Key Responsibilities
Provide direct patient care under the supervision of an RN or physician
Monitor residents' health and report changes to the charge nurse or physician
Administer prescribed medications and treatments accurately and on time
Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
Document nursing care and resident status accurately in medical records
Participate in the development and revision of care plans
Collaborate with interdisciplinary team members to ensure optimal care outcomes
Respond promptly to residents’ needs and emergencies
Educate residents and families on healthcare needs and wellness
Maintain a safe, clean, and positive environment for residents
Qualifications
Current and valid LPN license in [State]
CPR/BLS certification (or willingness to obtain upon hire)
Previous experience in a nursing home or long-term care setting preferred
Strong clinical and communication skills
Ability to work collaboratively and compassionately with elderly residents
Familiarity with electronic medical records (EMR) systems preferred
All Shifts Available! Full time & Part-time 8am-4pm, Part-time 4pm-12am, and Full time & Parttime 12am-8pm Rate: Negotiable
